HOW TO ARRANGE YOUR RECORDS TRANSFER:

Please click on the Medical Record Transfer request link above, this will direct you through to a digital MRT request form. Once you have filled out the MRT consent form, click ‘submit’. Allow between 7 to 10 business days for your record to be securely transferred to Mojo Klinik.

Note that a $40 administrative fee applies for all Medical Record Transfers. This fee helps cover the time and governance processes involved in securely preparing and transferring your medical file, including privacy protection and compliance with Australian medical record standards.

This fee will be collected at your first visit to see Dr Varipatis at Mojo Klinik, please do not call reception to pay, it will be added to your first appointment invoice at Mojo.

If you would prefer to continue your care with our clinic, you are very welcome to do so and no transfer is required. Reception is happy to speak with you regarding alternate care options.
